How to get started with HUD Housing

  • This ranch-style-open concept, fully accessible home is located in the city of Jefferson in a quiet residential neighborhood.   All bedrooms are good-sized private rooms, and the home has two full bathrooms. All appliances are included.  Just outside of the living room area is a deck.
  • There is a washer and dryer on the main floor of the home.  There is a full unfinished basement and a large backyard.
  • Individuals will undergo an assessment to determine if St. Coletta of Wisconsin can support the needs of the potential applicant as well a an assessment to determine the required intermittent staff support.
  • The needs of individuals can be: supported with housekeeping and organization, menu-planning, shopping (grocery and other), medical appointment assistance, medication monitoring and set-up, laundry and clothing care, community orientation, and safety, as well as money management.
  • Staffing support will include A case coordinator, a Residential Manager, Nursing Oversight, and Direct Support Professionals.
  • There is an interconnected fire alarm and a complete sprinkler system in the case of a fire.
  • There is 24/7 emergency on-call support.

   St. Coletta Transportation Services, taxicab, or other transportation services are available at an additional cost.

To qualify to live in this home, HUD must approve you for this income-based rental. You will complete the admissions process and qualify for admission to St. Coletta of WI. HUD establishes your monthly rent amount and security deposit based on your monthly income. This home is a Supported Living Home, meaning that individuals will be provided intermittent staff support as determined by assessment and as written in their ISP.

Beginning Admissions Process at St. Coletta*


Guiding you through HUD Application Process

  1. Reach out to St. Coletta’s Admissions Director. You can fill out an inquiry form or call for an admission packet.
  2. Fill out the admissions packet and return it with deposit.
  3. Schedule a tour
  4. Admission team review (weekly on Tuesdays).
  5. Assessment decision is made.
  6. Assessment is scheduled or the applicant is referred to another agency.
  7. Assessment team review.
  8. Receive and fill out, return the admission packet. A HUD Packet will be included if applicable.
  9. Set admission date.
  1. Receive Tenant Information Questionnaire (TIQ) from St. Coletta’s HUD Accountant.
  2. Receive verification forms to sign and return to HUD Accountant.
  3. St. Coletta’s HUD Accountant will begin verifying information listed on the TIQ (This process can take up to 6 weeks).
  4. After all information listed on the TIQ is verified HUD will decide on the eligibility.
  5. Fill out and sign form 50059 and the lease agreement.
  6. If approved, a form 50059 and the lease agreement will be sent out to be signed and St. Coletta’s HUD Accountant.
  7. Move in and meet new friends!
